Control of temporary works excavation
Poor, high-water content excavated material, slipped into a 15-20m deep borrow pit. The material had slipped to the bottom of the pit partially burying excavation, crushing and screening equipment. If this slip had occurred during working hours, the impact could have been very serious.

Incorrectly fixed sheets blow off parapet
This report concerns aluminium coping sheets that blew off the parapet of a block of flats under construction. The reporter says a fierce wind blew off several sheets which fell to the ground about 9m below. A building and a car were damaged but thankfully no one was hurt, although the potential for personal injury or death was significant.

When visiting a client during a period of unusually hot weather, it was noted that the vast majority of the building’s fire resisting door sets were wedged open in an attempt to increase ventilation and reduce internal temperatures.

A member of the public reported a partially collapsed retaining structure to bodies they believed could be the responsible authority. Despite continued efforts to get attention to the issue, there was no apparent action to address the concern. This raised the reporter’s concern for public safety.
