Glued on stainless steel panels fall
A ceiling in a retail setting was designed to be faced with reflective finish panels. The contractor proposed stainless steel bonded to MDF with folded edges and concealed mechanical restraint. The designer overruled the contractor and selected an adhesive-only solution which was implemented. Some months later, several panels failed, with some stainless steel facings falling to the floor.

Accidental partial demolition of a beam
During a site visit, the permanent works design engineer noticed that hydro-demolition of the end of a floor supporting beam had taken place out of sequence. This introduced a pattern of bending moments and shear forces that were not considered in the original design.

Painted faying surfaces leads to connections with insufficient load capacity
Painted, rather than the required unpainted connection faying surfaces, were identified by the Resident Engineer of a reporter's firm during the erection of a primary frame for a large project. The painted surfaces resulted in the connections not having sufficient load carrying capacity and necessitated remedial works.

Wall ties designed to connect masonry partitions to adjacent steelwork framing were not installed leaving the masonry walls unrestrained. Walls were seen to be rocking in high winds.
