During inspection of scaffolding prior to issuing a scaffold inspection certificate and permit to load, the Temporary Works Coordinator (TWC) observed lifting beams with end stops and ultimate stop cross bolts incorrectly installed. This was in direct contravention of the manufacturer's instructions.
Remedial measures were undertaken prior to the TWC issuing a scaffold inspection certificate and permit to load. According to the reporter, the TWC shared the message with colleagues to ensure that end stops and ultimate stop cross bolts were correctly installed in other projects.
