Stunts on bridges cause concern
A thrill-seeker travelling up and over the arch of a bridge, risking serious injury to themselves and others, raises the concerns of a reporter.

Understanding finite element analysis for pile caps
A reporter questions the validity of using two-dimensional finite element (2D FE) shells to model structural elements such as pile caps.

Control of temporary works excavation
Poor, high-water content excavated material, slipped into a 15-20m deep borrow pit. The material had slipped to the bottom of the pit partially burying excavation, crushing and screening equipment. If this slip had occurred during working hours, the impact could have been very serious.

Failure of cantilevered stone staircase
During the renovation and change-of-use conversion of a former minor stately home built around 1830, under pedestrian loading, a ‘cantilevered’ stone staircase partially collapsed without warning.

Concerns over execution class categorisation of steel pedestrian bridge
A reporter is concerned in respect of the steelwork execution class, and quality management arrangements, applied to the construction of a public access pedestrian bridge.

Connection fixity considerations for steel frame modelling
This report concerns the modelling of joints when using computer programs. In some cases, engineers are applying rotational releases to joints in models but subsequently designing the connections as rigid. This leaves a fundamental mismatch between analysis and design which may lead to unsafe structural connections argues a reporter.

Incorrect modelling of a cantilever
A steel framed building was incorrectly modelled and designed resulting in excessive deflection of part of the frame even before the frame was loaded with the self-weight of the building.

Further example of incorrect finite element modelling
Concerns in respect of the sufficiency of finite element modelling of masonry walls as part of a reinforced concrete framed building.

Apparently simple home improvement work leads to dangerous situation
Apparently simple home improvement work, consisting of reroofing and a loft conversion, resulted in a 'near miss' with potentially dangerous conditions for neighbours.

Modern methods of construction and robustness
This report relates to the design of a low rise modular building and the requirement, or otherwise, for positive fixings between the superstructure and the substructure.
