Failure of Firefighters lift to operate
The reporter tells of an instance when a Firefighters lift switch, located on the Fire Service Access Level (FSAL) in a multi-storey building failed to operate. After an investigation the switch was found not to have been connected.

Non compliant guard rails on high rise apartment buildings after modification

Misleading fire rating on hatches
Retrospective fitting of various amenities including TV aerials, broadband, and satellite connections, may result in new access points being made in walls, ceilings, or through floors. These openings need to be made good using a method that provides the required fire safety properties for that specific situation, including integrity, insulation, and resistance to smoke spread.

Unsafe instructions from a building inspector
On the site for an extension to a domestic property, a building inspector instructed a contractor to excavate a deep trench. The trench was in front of the neighbour's adjacent boundary retaining wall, and left a deep vertical face under the house being extended.
