CROSS-US welcomes reports about structural safety issues related to buildings and other structures in the built environment.

Who can report?

Our confidential reporting system can be used by professionals who work with buildings and other structures. This includes anyone with an interest in, or responsibility for, structural safety.

How reporting to CROSS-US works

  1. Submit a report

    You submit a report about a safety issue using our secure website. After you submit a report, you will receive a confirmation email. Designated People at CROSS-US will communicate with you as they process your report.

  2. Report is anonymized

    The Designated People will anonymize your report. This includes removing your personal information and any other identifiable details in the report. They will also edit the report for clarity.

  3. Expert Panel comments

    The anonymized report may be sent to the CROSS-US Expert Panel to add their comments. They use their experience to help professionals understand what can be learned from the report.

  4. Legal review

    A legal review of the anonymized report takes place to make sure there are no concerns with publishing the report.

  5. Review your report

    The final anonymized report is sent back to you for any comments.

  6. Report is published

    The anonymized report is published on the CROSS website for others to learn from. Your personal information will not be published with the report. After the report is published, all of the associated information, including your personal information, is deleted from our systems.

What can be reported to CROSS-US?

CROSS-US welcomes reports about structural safety issues related to buildings and other structures in the built environment.

Reports should aim to include information that will help others to learn from the safety issue identified.

Reports typically relate to concerns, near misses or incidents.

Reports do not have to be about current activities providing that the issue identified is still relevant. Your report might relate to a specific experience or it could be based on a series of experiences indicating a trend.

No concern is too large or small to report. Small scale safety issues are important because they can be the precursors to major safety incidents.

What should not be reported to CROSS-US?

The following types of reports should not be submitted to CROSS-US:

  • Reports that are not about structural safety issues related to buildings and other structures in the built environment

  • Reports that include any element of criminal activity

  • Reports where legal actions may be pending relating to the circumstances behind the report when the issues in a report mainly or solely concern personality conflicts

  • Reports where issues identified mainly or solely concern industrial relations matters and/or problems with terms and conditions of employment

  • Reports where issues mainly or solely concern occupational health or safety issues

If we receive reports that are not within our area of expertise, we will attempt to identify who the reports can be submitted to instead. You can find more information on our other safety reporting systems page.

Can I report a live or urgent safety issue to CROSS-US?

CROSS-US is not able to give advice and does not have any power to intervene in relation to specific projects or matters of concern.

If advice is needed in these circumstances, or you consider there to be a potential risk to health and safety and/or the environment, your first step should be to raise this with the organisations concerned if this is possible. If applicable, you should also speak to your line manager.

If this does not resolve the issue, or if the response you receive is inadequate, then you should inform the appropriate regulator. CROSS-US may be able to help you identify the appropriate regulator. In such cases, you should consider whether it is necessary to report the matter on a non-confidential basis.

You may be able submit your report to other safety reporting systems.

Can I report an individual or organisation to CROSS-US?

CROSS-US is not able to give advice and does not have any power to intervene in relation to specific projects or matters of concern.

Why are CROSS reports anonymized?

Designated People at CROSS-US anonymize all reports. This ensures that no individual, organisation, product, asset, or any other identifiable feature is included in a report. This does not cover information already in the public domain.

Anonymizing reports is an important part of the no blame culture of CROSS. It allows learning to be shared without creating concerns in areas like liability, public reputation, co-worker relations, client loyalty or insurance.

Why do CROSS collect personal information from reporters?

We collect the reporter’s personal information so that we can communicate with them as we process their report. This can include verifying the contents of their report.

The reporter’s personal information is not published with their report. It is deleted from our systems when their report is published.

Only Designated People at CROSS-US have access to the reporter’s personal information. They anonymize each report before it is shared with others in CROSS. You can read more about the Designated People on our CROSS-US team page.

Can I submit a report to CROSS-US offline?

While most people use our secure online system, you can post a report to us by downloading and completing our CROSS-US offline reporting form.

What you send us will only be opened by one of the Designated People at CROSS-US. They will treat everything you send in confidence.

 

How long will it take for my report to be published?

Once a report is submitted, it typically takes between 4 to 12 weeks before it is published on our website.

It may take longer if we have a lot of reports to process or if our Expert Panel need more time to review and comment on your report.

We will keep you updated on the progress of your report.
