Punching shear in concrete slabs at perimeter columns
A reporter raises concerns about the underestimation of bending moments in perimeter columns when assessing punching shear in adjacent concrete slabs. Punching shear failures can be brittle and catastrophic. Designers need to include the appropriate ultimate moment in the design actions.

The management of design related risks: structural, civil and fire engineers
Significant, and tragic, failures involving structures continue to occur: fires in apartment blocks in Milan, London and Taiwan, and structural collapses in Florida and Nigeria being recent examples. This paper gives emphasis to the need for engineers to consider risk management at a fundamental level.

This report draws attention to the excessive and incorrect use of standard details and notes on structural drawings, as well as the assumption that the builder/contractor will somehow work out the designer's intention of these on site.
