Concerns about punching shear in a flat slab
This report concerns potential shear failures in flat slabs. This originates from inconsistent design and detailing of the shear reinforcement where a proprietary shear link system is assumed in the design but not followed in detailing.

Inadequate brick ties in gable walls

Lack of fire safety competence in care home setting
A fire and rescue service is concerned about the lack of competence of maintenance personnel working in care homes. This is following two incidents where poor electrical and fire door maintenance put residents at risk.
