SCOSS has been concerned for some time at the use of structural fixings where these so called minor items have not received the attention they deserve given their safety critical nature.

Tension systems and post-drilled fixings
Lining failures occurred in the Boston (Big Dig) tunnel in 2006, in the Balcome rail tunnel in the UK in 2011 and in Japan’s Sasago tunnel (2012); all related to the use of unsuitable fixings.

Overloading a portal frame with suspended services
A reporter designing a single storey portal frame used a standard roof service loading of 0.25kN/m2. After the project was completed, a tenant for the unit took over fit out and added a considerable amount of sprinkler pipework into the roof. This created local overloading.
