Concern about rebending starter bars
This report concerns the bending and rebending of starter bars on site during the construction of a multi storey tower. The reporter considered that such a practice could compromise the structure under extreme load conditions.

Cladding subcontractor change of connection strategy led to failure
As part of their final design submission, a cladding subcontractor changed the connection strategy for their cladding system. The project structural engineer did not identify the design change and the cladding loading resulted in excessive deflection and some twisting of the primary perimeter steel.

Glued on stainless steel panels fall
A ceiling in a retail setting was designed to be faced with reflective finish panels. The contractor proposed stainless steel bonded to MDF with folded edges and concealed mechanical restraint. The designer overruled the contractor and selected an adhesive-only solution which was implemented. Some months later, several panels failed, with some stainless steel facings falling to the floor.

Overloading a portal frame with suspended services
A reporter designing a single storey portal frame used a standard roof service loading of 0.25kN/m2. After the project was completed, a tenant for the unit took over fit out and added a considerable amount of sprinkler pipework into the roof. This created local overloading.

Masonry panels rock in wind due to missing wall ties
Wall ties designed to connect masonry partitions to adjacent steelwork framing were not installed leaving the masonry walls unrestrained. Walls were seen to be rocking in high winds.
