Concrete system buildings - need for period inspection
Evidence of corrosion of tendons in post-tensioned components came to light which was associated with exposure of tendons to moisture in voids in ducts and sometimes also the presence of chlorides.

Tension cable and rod connectors
In a number of instances, suspension fork connectors for high strength steel bars (cables or rods) acting in tension have failed at loads well below the intended design capacity or have been found to be substandard either during construction or in service.
