Concerns about punching shear in a flat slab
This report concerns potential shear failures in flat slabs. This originates from inconsistent design and detailing of the shear reinforcement where a proprietary shear link system is assumed in the design but not followed in detailing.

Tower crane base designs
A reporter’s firm has been involved in the design and checking of numerous tower crane bases and have come across many examples where the supplier of the tower crane has supplied incorrect loadings. These incorrect loadings could have led to crane failures had the errors not been spotted.

Connection fixity considerations for steel frame modelling
This report concerns the modelling of joints when using computer programs. In some cases, engineers are applying rotational releases to joints in models but subsequently designing the connections as rigid. This leaves a fundamental mismatch between analysis and design which may lead to unsafe structural connections argues a reporter.
