Unsafe instructions from a building inspector
On the site for an extension to a domestic property, a building inspector instructed a contractor to excavate a deep trench. The trench was in front of the neighbour's adjacent boundary retaining wall, and left a deep vertical face under the house being extended.

Contractor uses incorrect fixing bolts for masonry support angles
A correspondent noticed that proprietary masonry support angles specified to support the outer leaf of masonry had been incorrectly connected to the steel beam using carriage bolts.
