Overloading a portal frame with suspended services
A reporter designing a single storey portal frame used a standard roof service loading of 0.25kN/m2. After the project was completed, a tenant for the unit took over fit out and added a considerable amount of sprinkler pipework into the roof. This created local overloading.

Incorrectly installed tension control bolts could have led to serious consequences
A reporter finds incorrectly installed system HRC preload bolt assemblies, better known as tension control bolts, during a site inspection of a new highway bridge over a railway. The incorrectly fitted bolts could have had major structural implications, and led to other safety risks, if the installation error had not been spotted.

Concerns over corrosion of dissimilar metals in fixing cavity wall tie systems
A reporter is concerned about corrosion that may occur between dissimilar metals that can be used in cavity wall tie systems. The reporter has noted that stainless steel and carbon steel may be fixed together in wall cavities, which are damp environments, and that corrosion of dissimilar metals in contact may occur.

Glued on stainless steel panels fall
A ceiling in a retail setting was designed to be faced with reflective finish panels. The contractor proposed stainless steel bonded to MDF with folded edges and concealed mechanical restraint. The designer overruled the contractor and selected an adhesive-only solution which was implemented. Some months later, several panels failed, with some stainless steel facings falling to the floor.
